Book a consultationWhat it is
The flat piercing sits on the wide plane of upper-ear cartilage.
A flat piercing goes through the broad, flat expanse of cartilage on the upper ear — the open panel between the outer helix rim and the inner antihelix ridge, below where a helix would sit. It is a cornerstone of a built-out ear because it lives on flat tissue with room for a decorative flat-back stud that lies clean against the skin.
Single flat
One stud on the upper-ear plane
The standard placement. A single flat-back labret stud set on the flat cartilage panel, usually toward the upper portion of the ear. The stable base tissue makes it one of the more forgiving cartilage placements to heal when it is left alone.
Double / triple flat
Two or three studs in a line or cluster
The flat plane has enough real estate for a small constellation. Apollo often stages these across appointments — a second or third stud placed once the first is settled — so each opening heals cleanly and the spacing reads as intentional.
Flat in a curated ear
Anchoring a larger composition
A flat frequently anchors a curated ear alongside a helix and a conch. The flat panel gives the piercer a stable, visible plane to seat a feature gem or cluster that carries the whole arrangement.
Flat vs. helix
Where people mix them up
A helix sits on the curved outer rim; a flat sits on the open panel just inside that rim. The difference matters for jewelry: a flat almost always takes a flat-back stud, while a rim helix can also take a ring once healed.
Cartilage keeps its own calendar. A flat is not slow-healing because something is wrong — it is slow because that is how cartilage works.
The flat plane is the best canvas in the ear for a feature gem. We place it to sit flush, so the stud reads as jewelry, not hardware.
Every Apollo piercer is Fakir-certified. On cartilage, technique and jewelry quality are the whole game.
Pain & honesty
An honest read on how a flat piercing feels.
Cartilage has more bite than a lobe and less than most people fear. Here is the straight version, without the marketing gloss.
The pass itself
A firm, quick pop with real pressure — most clients land it around a 5 to 6 out of 10 for one to two seconds. It is sharper than a lobe because the needle is passing through dense cartilage, but it is over almost as soon as it begins.
The first few days
A dull throb, warmth, and tenderness to the touch for three to five days. Sleeping on that side is the main day-to-day irritation. This is expected cartilage behavior, not a sign of trouble.
The bumps question
Irritation bumps are the most common flat-piercing complaint — usually from snags, sleeping pressure, or jewelry that is the wrong length. They are manageable and preventable, and they are not the same as infection. Come back in and we adjust.
What is not normal
Spreading redness, hot swelling, thick discharge, or fever. We do not diagnose — we refer you to a physician. Most of what clients panic about is ordinary cartilage grumpiness that settles with clean aftercare.
Healing timeline
Five phases. Cartilage rewards patience.
A flat piercing surface-heals in months, not weeks. Rushing the jewelry change is the fastest way to restart the clock.
Jewelry standards
What Apollo puts in a fresh flat piercing.
The flat-back labret stud is the correct piece for this placement. The metal and the fit are not negotiable on cartilage.
Flat-back labret stud
A post with a flat disc backing that sits smooth behind the ear — comfortable to sleep on and impossible to snag from behind. This is the standard install piece for a flat piercing.
Implant-grade titanium (ASTM F-136)
The default install metal at Apollo. Nickel-free, biocompatible, MRI-safe. On fresh cartilage the metal has to be flawless — titanium is the professional-studio standard for a reason.
Solid 14k / 18k gold
Nickel-free solid gold, never gold-plated. Available at install when requested and stocked, and a common upgrade once the piercing is settled. See our sensitive-skin jewelry notes if you react to metals.
Threadless or internally threaded
The threading stays on the inside, so nothing rough passes through the healing channel. External threading drags across raw tissue and is one of the quiet causes of irritation bumps. Apollo does not install it fresh.
Correct post length
The install post is slightly long to allow for early swelling. Once that settles, a downsize to a shorter post keeps the end sitting close to the ear — a long post that rocks and catches is the number-one bump generator.
No rings at install
A flat piercing is a stud placement. Rings do not belong in fresh flat cartilage — they move, catch, and prolong healing. Decorative ends and gems come after the channel is stable.
Method
How Apollo performs a flat piercing.
Single-use sterile needle, a marked-and-mirrored placement, and Fakir-certified piercers working under California's body-art code.
Needle, never a gun
Every flat piercing is done with a single-use sterile hollow needle. Piercing guns crush cartilage and cannot be sterilized — they have no place near an ear. The needle removes a clean core and sets up a cleaner heal.
Marked, mirrored, consented
The piercer maps the placement on the flat plane, accounts for your existing and planned piercings, and you approve the dot in the mirror before anything happens. Placement on the flat is a composition decision, not a guess.
Fakir-certified piercers
Apollo's piercers hold Fakir Intensives certification — the deepest formal training in the trade. On cartilage, where technique decides whether a piercing heals straight and bump-free, that training is the difference.
California Safe Body Art Act
Registered with LA County. Autoclave sterilization, sealed sterile jewelry opened at the chair, sharps-containered disposal, full documentation. The same standard on every piercing we perform.
Aftercare
Six habits that decide how a flat piercing heals.
Cartilage aftercare is mostly about leaving it alone. The clients who heal cleanest are the ones who do the least to it.
Sterile saline, twice a day
Spray or soak with sterile wound-wash saline morning and night. That is the whole cleaning routine. No alcohol, no peroxide, no tea tree oil, no ointments — those all irritate cartilage.
Do not sleep on it
Pressure while you sleep is the leading cause of flat-piercing bumps and crooked healing. Sleep on the other side; a travel pillow with a hole, or a clean folded T-shirt, saves the side you cannot avoid.
Hands off, no twisting
Do not rotate the jewelry, do not pick crust, do not play with it. Twisting a cartilage piercing tears the forming channel. Touch it only with clean hands and only to clean it.
Mind hair, phones, and masks
Hairbrushes, over-ear headphones, phone screens, and mask straps all snag a flat stud. Tie hair back, wipe your phone, and be deliberate around the ear for the first few months.
Come in for the downsize
Once early swelling is gone, book a downsize to a shorter post. It is a quick visit that prevents months of snag-driven irritation. This is standard Apollo practice, not an upsell.
Give it the full timeline
Do not change the jewelry yourself and do not call it healed at week six. A flat piercing needs four to nine months. Bring it to us for the first jewelry change and we will tell you honestly if it is ready.
Common mistakes
Six patterns that ruin a flat piercing.
Nearly every disappointing flat-piercing outcome traces to one of these — and every one of them is preventable.
Sleeping on it
The single most common cause of bumps and off-angle healing. Fix: protect the side, use a travel pillow, and give it months not weeks.
Chasing bumps with harsh products
Tea tree oil, alcohol, and aspirin paste make irritation bumps worse. Fix: plain sterile saline, remove the source of the snag, and come in so we can check the fit.
Changing jewelry too early
Swapping the stud at six weeks tears a channel that is far from healed. Fix: leave the install piece in and let us do the first change once it is genuinely ready.
Skipping the downsize
A long post rocks and catches for months. Fix: book the downsize once swelling is gone — it is a five-minute visit that prevents most bumps.
Cheap or external-threaded jewelry
Mystery-metal studs and external threading are dermatitis and snag generators on fresh cartilage. Fix: implant-grade titanium or solid gold, threadless or internally threaded, only.
Stacking too much at once
Two or three fresh flats in one sitting compounds swelling and pressure. Fix: stage a multi-point flat or a full ear curation across visits so each opening heals clean.
FAQ
The questions every flat-piercing consultation surfaces.
Seven questions on pricing, pain, healing, jewelry, first-timers, the helix comparison, and sleep.
How much does a flat piercing cost in LA?
Apollo quotes a flat piercing at consultation because the price moves with the jewelry you choose. A plain implant-grade titanium flat-back stud is the entry point; solid gold and gem or cluster ends cost more. We price the specific piece and placement rather than posting a flat rate, and we will always tell you the number before the needle comes out.
Does a flat piercing hurt?
It is a firm, quick pop — most clients rate it a 5 to 6 out of 10 for one to two seconds, sharper than a lobe because it passes through dense cartilage. The pass is brief. The bigger reality is the first few days of dull throbbing and tenderness, especially when you try to sleep on that side.
How long does a flat piercing take to heal?
Four to nine months for most people. It will look calm on the surface within weeks, but cartilage heals slowly from the inside, so it is not done when it looks done. Do not change the jewelry yourself — bring it to us for the first change and we will confirm it is genuinely ready.
What jewelry is used for a flat piercing?
A flat-back labret stud — a post with a smooth disc backing that sits flush behind the ear and is comfortable to sleep on. Apollo installs implant-grade titanium (ASTM F-136) by default, with solid 14k or 18k gold available. Threadless or internally threaded only; no external threading, no gold-plate, and no rings in a fresh flat.
Can a flat piercing be my first piercing?
Yes. A flat is a stable, forgiving cartilage placement and a fine first piercing for an adult who is ready for a multi-month heal and disciplined aftercare. If you would rather start simpler, our lobe and helix options are gentler on the timeline — the first-piercing guide walks through the trade-offs.
What is the difference between a flat and a helix piercing?
A helix sits on the curved outer rim of the upper ear; a flat sits on the open panel of cartilage just inside that rim. They heal on the same cartilage timeline, but a flat almost always takes a flat-back stud, while a healed rim helix can also wear a ring. In a curated ear the two often sit side by side.
Can I sleep on a flat piercing?
Not while it heals. Sleeping pressure is the number-one cause of irritation bumps and off-angle healing on a flat. Sleep on the opposite side and use a travel pillow with a hole or a clean folded shirt to keep weight off the ear until it is fully settled — often nine months or more.
